At Burton and Uttoxeter Live at Home scheme, we want to enable older people to maintain their independence and live later life well.

Our Live at Home community group provides a whole range of support and services for older people that are available to older people across Burton and the surrounding area, including a wide range of social activities and events, supported outings, befriending services for older people, interest groups, signposting and other services that enable older people to remain actively involved in their local community. 

Services offered at Burton Live at Home scheme include:

  • Befriending (1:1 and telephone)
  • Phone checks
  • Trips and outings
  • Social groups
  • Dementia groups
  • Arts and crafts groups

The types of service and support we offer puts members in control of their own support plan and members can choose the types of activities or services they would like, providing them with flexibility and variety.

We do this by offering different types of service that suit different wants and needs that members can access, picking and choosing from the whole range.

If you’re looking at Live at Home to help someone you know, we understand. With a busy work life, family and other responsibilities, sometimes it just isn’t possible to travel regularly and give a loved one the dedicated care and support you wish you could provide.

Information for families and carers

If an older person in hospital has no family, friends or neighbours, we can assist them to return home. We work with GP practices, hospital discharge and re-enablement teams and Clinical Commissioning Groups (CCGs) to support older people adjust and settle back at home after a period in hospital.

Through our preventative, person-centred services, we can reduce the strain on local health services – delaying, minimising or even preventing people’s need for them.
