If you’re looking at Live at Home to help someone you know, we understand. With a busy work life, family and other responsibilities, sometimes it just isn’t possible to travel regularly and give a loved one the dedicated care and support you wish you could provide. Refer a family member or friend

Information for healthcare professionals

If an older person in hospital has no family, friends or neighbours, we can assist them to return home. We work with GP practices, hospital discharge and re-enablement teams and Clinical Commissioning Groups (CCGs) to support older people adjust and settle back at home after a period in hospital. Refer a patient
