Providing support and assistance to enable older people to find independence again.
If an older person in hospital has no family, friends or neighbours, we can assist them to return home. We work with GP practices, hospital discharge and re-enablement teams and Clinical Commissioning Groups (CCGs) to support older people adjust and settle back at home after a period in hospital.
On the day of discharge, MHA Communities can organise for volunteers to meet an older person at their home with a bag of shopping of essential goods for their first two days out of hospital and ensure that they are safely back in a warm home environment.
Our volunteer would continue to befriend the older person with shopping trips, picking up prescriptions etc. for a period of about six weeks. To encourage and promote ongoing independence and avoid further spells in hospital, we would encourage the older person to become a member of their local MHA Communities group or join our digital offering and benefit from all our services.
