We are always happy to support with social worker referrals, GP referrals, district nurse referrals and community referrals. We can also provide self-help leaflets, posters and useful resources for GP surgeries, helping to reduce unnecessary appointments. 

Professional referrals in 5 simple steps

  1. Find a suitable care home for your client or patient by using our care home finder below.

  2. Contact the care home directly by phone or email to check availability and arrange a visit. You can find the contact details on each care home webpage.

  3. If the home has availability to meet your client or patient's care needs, they will arrange an assessment. You will need to provide a copy of their latest assessment and care plan, as well as any other relevant documents.

  4. The home will review your referral and contact you within 48 hours to discuss the next steps.

  5. If a place can be offered, the home will support you to complete any necessary application forms and financial assessments. Top-up fees can be arranged directly with the family.
